  • Call the toll-free service center at 1-800-800-3937 (EYES) and give your member ID number #FIS-1422. Both numbers are on your Coast to Coast ID card. Phones are answered Monday-Friday, 8 a.m.-6 p.m. (Central Time).
  • Ask for the location of the nearest participating Coast to Coast Vision provider. There are several area locations to choose from.
  • When you arrive at the store or office, present your Coast to Coast ID Card; it provides coverage for your entire family.
  • Select the eyewear of your choice and receive your discount at the time of purchase. No claim forms are required.
  • Contact lenses are available by mail at discounted rates by calling America’s Eyewear at 1-800-878-3901.
• Your Eyewear Guarantee—When you purchase eyewear through Coast to Coast Vision, low prices and satisfaction are guaranteed. If you find a lower price on identical eyeglasses within 30 days of your purchase, the difference will be refunded. Plus, if you are not satisfied with your eyewear purchase for any reason, the entire purchase price will be refunded. • Rx Advantage™ Prescription Drug Program—With the price of drugs these days, the cure can be more painful than the illness. But you can ease the pain in your wallet when you carry an Rx Advantage Card.
  
  When you use this card at any participating pharmacy, you can get significant savings on prescription medications — brand name and generic. And you’ll rest easier knowing that our on-line monitoring network informs your pharmacist of your drug history and how much time has passed since your last prescription was filled. The network also alerts your pharmacist to the possibility of interaction with other medications. To receive your Rx Advantage savings:
  • Order your Rx Advantage Card. Complete the order form in your Vitality Plus Gold membership kit and mail it in. You can choose individual or family participation -- there’s no extra charge.
  • You’ll receive your Rx Advantage Card and membership packet within four weeks. If you choose family participation, you’ll receive two cards.
  • Locate one of the participating pharmacies in your area. Just call toll free 1-800-251-0003. If you’re away from home, call the toll-free number to find a participating pharmacy near you. You can use any pharmacy in the Rx Advantage nationwide network. Be sure to carry your Rx Advantage Card with you at all times, because you must show your card every time.
  • Show your Rx Advantage Card (not your Vitality Plus Gold card) when you present your prescription — before the prescription is filled. Your pharmacy will key in your number and receive your preferred pricing in about 30 seconds. The system also checks for possible interactions with other medications you may be taking and alerts your pharmacist. If the pharmacy’s regular price is lower than the Rx Advantage preferred price, you will be charged the lower price.
  • When your prescription is filled, pay the total price and you’re on your way.
  • Please note that RxAdvantage is a discount card, not a prescription drug plan. You should be able to use it with your Medicare Part D prescription drug plan, if you have one. Ask your pharmacist.
• Hearing Aid Services—Vitality Plus Gold members can receive savings at local hearing specialists. Here’s a sample of what some participating providers offer:
